S.A.F.E.® Customer Submission Form

The S.A.F.E. customer submission form can help Retirement Plan participants and IRA customers contact Voya with an immediate concern about fraud if the call center is closed. Generally, the call center is open for most plans and products from 8 a.m. to 9 p.m. ET. Please use the toll-free number on your most recent statement or call 800-584-6001 if you are unsure.

If you have a fraud concern, and the call center is closed, you should contact Voya using this S.A.F.E. form.

Please use this form to:

  • Report suspected fraudulent access to your account, including changes to contact information, withdrawals or attempted withdrawals
  • Report receipt of unexpected multifactor codes or other notifications
  • Inquire on security best practices, especially if you have been a recent victim of identity theft or account takeover elsewhere

Please do not use this form to:

  • Request service on your account, including transaction requests, website access assistance, etc. Please contact the call center for normal service needs.
  • Express dissatisfaction with any service you have received. If you have an unresolved service need, please contact the call center, as a first step.
