Exclusions and Limitations for the Certificate, Spouse Critical Illness Insurance Rider and Children’s Critical Illness Insurance Rider are listed below (these may vary by state). Benefits are not payable or are reduced for any loss based on the following provisions.
Pre-existing Condition Limitation A pre-existing condition means a sickness which, within the 12-month period prior to coverage effective date or any increase in coverage for each covered person, resulted in the covered person receiving medical treatment, consultation, care or services (including diagnostic measures). For the first 12 months following the coverage effective date or any increase in coverage for each covered person, benefits are not payable for treatment resulting from a pre-existing condition. If the treatment begins more than 12 months after the coverage effective date or an increase in coverage for the covered person, benefits are payable for any treatment even if resulting from a pre-existing condition.
Age Reductions The employees and spouse's critical illness benefit amount and total maximum benefit amount will reduce to 50% on the employee's 70th birthday. Premiums do not reduce.
In New York, benefits are not payable for any specified disease caused in whole or directly by any of the following*
- Participation or attempt to participate in a felony or being engaged in an illegal occupation
- Suicide, attempted suicide or any intentionally self-inflicted injury
- War or any act of war, whether declared or undeclared
In New York, a pre-existing condition is a sickness, injury or physical condition for which you received medical treatment, consultation, care or services (including diagnostic measures) during the 6 months prior to your coverage effective date. For the first six months after your coverage effective date or any increase in coverage, we will not pay benefits if the date of diagnosis for any condition or illness is the result of a pre-existing condition. After the first six months of coverage or any increase in coverage, benefits are payable for any eligible for critical illness even if resulting from a pre-existing condition.
