Consumer Disclosure Regarding Receiving and Signing Documents Electronically
Please read the following information. By agreeing to the terms contained in this disclosure and proceeding with this transaction, you are agreeing that you have reviewed the following consumer disclosure information and consent to utilize electronic signatures and send and receive electronic communications with respect to this transaction, in lieu of using paper documents. This electronic signature service is provided on behalf of clients, by Voya Retirement Insurance and Annuity Company, Voya Institutional Plan Services, LLC, and/or other Voya affiliates (hereinafter, “Sending Party”) who is sending electronic documents, or requesting electronic signatures to you.

Please note that these disclosures apply only to allowing you to electronically receive and sign documents for this transaction, and does not constitute a request for electronic statement and confirmation delivery, asset movement, or account maintenance instructions. Additional forms and disclosure are required to effect such instructions, and may be provided by your plan sponsor and/or financial professional.

You are not required to receive or sign documents electronically. If you prefer not to do so, you may request to receive paper copies and withdraw your consent at any time as described below.

Guided Signing Experience
To make the electronic signing process intuitive and efficient, Sending Party offers a guided signing experience that you may choose to use. If you use the guided signing experience, you will be guided directly to areas of the document that require your signature and/or other required actions. This will skip certain sections or pages that do not require your signature or other action. Please Note: The document(s) that you are electronically signing contain important information about the transaction into which you are entering, and affect your legal rights. Accordingly, it is important that you review all of the document(s) in their entirety prior to finalizing all of your signatures. If you have any questions about the documents, please discuss with Sending Party.

Paper Copies
You are not required to receive or sign documents electronically and may receive and/or sign paper copies of documents if you prefer to do so. You also have the ability to download and print any open or signed documents sent to you through the electronic signature service using the PDF and Print icons. Adobe may also email you a PDF copy of all agreements you sign using the electronic signature service. If you wish to receive paper copies in lieu of electronic documents you may close this web browser and request paper copies from the Sending Party by following the procedures outlined below. Sending Party may apply a charge for requesting paper copies.

Use of the Service requires a standards-compliant web-browser which supports the HTTPS protocol, HTML, and cookies. Viewing PDF documents requiring additional software such as Adobe Reader or similar.

Withdrawal of Consent
You may withdraw your consent to receive electronic documents at any time. In order to withdraw consent you must notify Sending Party that you wish to withdraw consent and to provide your future documents in paper format. At any point after withdrawing consent you again consent to utilize the electronic signature system, you are once again consenting to receive documents relating to a particular transaction electronically. You may withdraw consent to receive electronic documents and utilize electronic signatures by following the procedures described below.

Scope of Consent
You agree to receive and sign documents electronically provided by Sending Party to you over the course of the transaction into which you are entering. You may at any point withdraw your consent by following the procedures described below.

Requesting paper copies, withdrawing consent, and updating contact information
You have the ability to download and print any documents Sending Party sends to you through the electronic signature system. To request paper copies of documents, or withdraw consent to receive and/or sign documents electronically please contact the representative of the Sending Party with whom you are working to effect the transaction by telephone or e-mail at the telephone number and e-mail address provided. If utilizing e-mail, please provide use the following subjects:

“Requesting Paper Copies” Please provide your name, email, telephone number, postal address and document title. Sending Party will provide you with the document referenced in your e-mail in paper format.

“Withdraw Consent” Please provide your name, email, date, telephone number, postal address. This will withdraw consent to receive and sign documents electronically for the particular transaction for which you consented to receive and sign documents electronically.

“Update Contact Information” Please provide your name, email, telephone number and postal address. This will update the contact information that Sending Party uses with respect to your receipt and signing of documents electronically.

Any fees associated with sending paper copies or withdrawing consent will be determined by the Sending Party.
